My brother's doctor has a deal with his 500 patients. $2,000 all visits covered, and has 24 hr access to his doctor. He pays that out of his HSA. Then he buys a policy for the catastrophic stuff.

A few doc's are moving to a cafeteria plan. We're evaluating our health benefits for next year for our firm, and we're estimating a raise of anywhere from 10-20% for the employer premiums... and in most cases that increase is passed on to the employee.
I foresee a ton of lawsuits from people who work 24 or 29 hours a week (or whatever the government deems "full time" to be by the time 2014 rolls around). Many employers will hire more people but not allow them to get to full time status so that they don't have to cover health premiums. Another strategy is to take the penalty for both the employer and employee, but give the employee a bonus.
